University of Wisconsin - Parkside Institutional Review Board
This committee provides institutional review for the protection of human subjects in research projects. The University of Wisconsin-Parkside has filed a Federalwide Assurance with the Federal agency that now oversees the conduct of research involving human subjects, the Office for Human Research Protection, or OHRP. OHRP’s home page is: http://www.hhs.gov/ohrp/. There, you can find the Federal regulation that UW-Parkside must abide by, 45 CFR 46, at: http://www.hhs.gov/ohrp/humansubjects/guidance/45cfr46.html
The Federalwide Assurance means that that UW-Parkside has pledged that all research involving human subjects will be reviewed by the Institutional Review Board if it is:

The procedure to request approval of your human subject research is as follows:
You will be notified by email regarding the approval of your project. You will also be provided a pdf copy of the fully executed request form for your records.
To be absolutely sure you don’t run into any obstacles, you would be safest if you give yourself AT LEAST ONE FULL MONTH before you need to start your research; you CANNOT start data collection without having already received IRB approval, or designation as exempt research.
WHAT HAPPENS IF MY PROJECT NEEDS TO LAST MORE THAN ONE YEAR?
The Federal Common Rule specifies that ongoing research has to be reviewed at least once a year. If more than minimal risk to subjects is involved in the research, or for any other reason, the IRB could in fact specify that the project must be reviewed in less than one year. About 30 days before the end of the initial review period, investigators doing non-exempt research will receive notification that their approval is about to expire. If the research is still active, then they should reply with a request to renew the approval. This is the investigator’s responsibility to pursue. If he or she fails to respond before the approval lapses, the investigator must cease any ongoing research interventions with subjects and must not accrue any more subjects to the project.
The Institutional Review Board, which OHRP calls IRB00000939 U Wisconsin-Parkside IRB#1, reports directly to the Chancellor. It is composed of 5 faculty, one of whom is the Chair, plus Academic Staff and Community representation.
