Which form do I use?
Batch DARS Request Form - This form should be used when larger volumes of Degree Audit Reports are needed and the user wants to leverage the scheduling, printing and administration capabilities of the UWP Computer Center in processing these larger volume requests.
Job Run Request Form — This form is used to request a previously created batch process be run on a specified date. These requests may vary from generation or processing of SCAFE (student course and faculty evaluation forms), to printing labels.
Issue Form — Issue forms are used to submit general questions, concerns or confusion related to any existing business process, policy, procedure or system that requires clarification, discussion or analysis of potential impacts.
Program Move Request Form — This form is used to document the migration of application systems or technical component changes from one environment to another. The form details what technical components are required to be moved together for the system or process to be tested or put in service in the target technical environment.
This form provides control and audit trail of system changes. This is a general purpose form and covers all types of moves performed including PeopleSoft, Network Servers, DARS, RDS, and others. The form also provides for verification of test results, instructions for special requirements in the move process, security access instructions, and in the case of non PeopleSoft moves, back-out & recovery instructions.Project Request Form — A project request form is used to request any new or enhanced systems functionality or for converting an existing process or system from one technical environment to another. Examples include: New Project - TutorTrack Package Implementation Request, Enhancement Project - Transcript Hold Flag Request, Conversion Project - Telecom Billing System Conversion Request.
RDS Access Request Form — RDS access request forms are used to request any of the following: creation of an RDS account, acquisition, setup and configuration of Brio and/or Crystal Reports Software on a users PC, and RDS Data Owner’s approval to access specific data tables, views and fields.
Test Scoring Request Form — The test scoring request forms are used to request pre-printed test forms for individual classes and to have the tests scanned and scored.
