Fleet Car Rental Policy
In need
of a rental vehicle from Facilities Management? Our department
would like be there to provide you with the best service possible. To
ensure that, the following (2) two items must be submitted and
approved prior to your departure. If you have any questions,

2. DRIVER AUTHORIZATION FORM available online: http://www.uwp.edu/departments/safety/files/drivauth.htm

A completed Car Fleet Form must be submitted 48 hours prior
to the
date of departure for your reservation
to be valid. This form must be complete and include the correct funding
string, and the name/names of the person/persons who will actually
be doing the driving. Car Fleet Forms are available on the Facilities
Management Web Page. On-line forms will be confirmed with an email
to the address on the top of the form. If the on-line form is being
submitted for a reservation within the next 48 hours, please call 2576
to check the availability of a vehicle. If a vehicle is not available,
a non-availability form will be sent to the email address on the form.
- You
must have the approval of the UW-Parkside Risk Management Department
in order to drive any state vehicle. To apply, complete
the Driver Authorization Form and submit it to Risk Management.
Drivers who do not meet the DOA established Minimum Driving
Standards will be reimbursed for miles on their personal vehicle
at the lower rate of .28 per mile.
Questions regarding reimbursement should be referred to Business Services.

You must submit electronically or present in person, your Car Fleet
Form at
least 48 hours in advance
to guarantee a reservation. We will not hold a reservation until we
have the confirming Car Fleet Form.

Please remember that we also have a NO SHOW/CANCELLATION POLICY which state that
any vehicle not picked up
as previously arranged, without 24 hours notice,will be charged a penalty
of one full days rental.
