Site Supervisor Forms
Below are the evaluation forms for
individuals who are supervising UW Parkside students in a SMFT major fieldwork
experience. Please complete the 'Site Supervisor Mid-Experience Evaluation
Form' half way through the experience. Then review your evaluation with
the student prior to sending it to the supervising UWP advisor. When the
filedwork is complete, please complete the 'Site Supervisor Final Evaluation
Form', review it with the student and send to the supervising UWP Advisor.
