Need a Duplicate W2 Form?
You may request a duplicate W2 form on UW-Madison's website beginning on February 15. Please click here to request a W2. Duplicate W2 forms for 2007 are free, but there is a charge for prior years.
Duplicate W2 forms will be mailed to your address in approximately 10 business days. If you need expedited service, please contact the Human Resources office at 595-2204.
