- Proposed UW-System unclassified title description, including title code
- Job Description in ADA Format (MS Word document)
- Title Review Cover Sheet and UWPQ for New Appointments - (MS Word Document)

- Justification for using academic staff title (UPG #7)
Existing Appointments - The following documents are required, must be submitted together and labeled appropriately:
- Proposed UW-System unclassified title description, including title code
- Title Review Cover Sheet and UWPQ for Existing Appointments - (MS Word Document)
or Title Review Cover Sheet and UWPQ for Existing Appointments - (MS Word Document)
- Past Job Descriptions
- Current Resume or Vita
- Supporting documentation of job performance, including a statement of rationale from the employee and/or supervisor
