New Appointments - The following information must be attached and labeled appropriately:

1. Proposed UW-System unclassified title description, including title code
   
   
2. Job Description in ADA Format (MS Word document)
   
   
3. Justification for using academic staff title (UPG #7)

4. Supporting documentation of job performance, including a statement of rationale from the employee and/or supervisor
