Address Change Information
You must contact the UW-Parkside Payroll office at 262-595-2220 to discuss the insurance and benefit changes that must be made. A checklist with detailed information will be sent to you at that time.
Two forms must also be completed:
- W-4 Form - complete and return to the UW-Parkside Payroll Office
- UW-Parkside Employee Data Form - complete and return to the UW-Parkside Payroll Office.
