Overview
College life is hectic enough without having to deal with the financial difficulty involved with an unexpected illness, injury or emotional stressor. Without good information and adequate health insurance, a minor incident can explode into a major problem. Using the Student Health & Counseling Center and having health insurance are both smart moves for students who want to avoid unnecessary financial, physical and emotional stress. Some students may still be covered by their parents’ insurance plan. It is wise to check with the company to assure the coverage requirements.
University of Wisconsin-Parkside participates in the UW System Campus Consortium Insurance plan administered by the Student Assurance Services, Inc. It is a voluntary health insurance plan available to domestic and international students, their families and children.

Eligibility
All registered students taking a minimum of 5 credit hours and all graduate students are eligible to enroll and purchase the insurance plan.

Intercollegiate sports insurance is an additional premium. Premium amount is $300.00.
Semi-Annual, Quarterly and Semester premium payments are available. Visa and MasterCard accepted. Intercollegiate sports coverage and rates, on a voluntary basis, are available and will be provided on the enrollment application.

INSURANCE INFORMATION YOU NEED TO KNOW...
The insurance can be purchased annually, semi-annually, quarterly or by the semester.
The insurance does send renewal reminders. However, be aware that they send it to the address you provided initially. It is your responsibility to report address changes to the insurance company. Renewals are ultimately your responsibility so mark your calendar to prevent a lapse of coverage.
The insurance is a "preferred provider" policy (see page 6 of brochure) which means there are certain medical facilities and physicians which will provide you with an 80/20 split on costs of covered benefits. To locate a preferred provider visit http://www.healtheos.com

- search for provider by using zip code and 50 mile radius
If you go to an "Out of Network" (someone not on the Preferred Provider List) the insurance will only pay a 60/40 split ON THE BENEFITS.
It is important that you read your insurance booklet, especially those items that are NOT COVERED (THE EXCLUSIONS AND LIMITATIONS SECTION).
If you have reason to file a claim, print a copy of the claim form which is on-line at http://www.sas-mn.com/claimform download.html, make a copy of the material (invoice, etc.) and send the original in to the insurance company at: Student Assurance Services, P. O. Box 196, Stillwater, MN 55082-0196. Keep a copy for your records, documenting the date sent in and any other pertinent information.
YOU MUST RESPOND to any correspondence you receive from the insurance company promptly. Failure to do so within the required timeframe can result in forfeiture of coverage for that claim.

The policy number is on the membership card.

DISCOUNT CARD
In addition to the insurance policy benefits you may purchase a Discount Card for $30.00 from the Student Assurance Services, Inc. /Best Benefits program. Or call 1-800-308-0374. This plan provides discounts on prescriptions, dental services and vision products.
INTERNATIONAL STUDENT INSURANCE PLAN
International students/scholars who are in the country on a non-immigrant Visa are mandated to have international health insurance coverage that includes medical evacuation and repatriation as long as they are students. The international student plan with Student Assurance Services, Inc. International Plan provides the services required at a reasonable cost. Parkside international students are automatically enrolled in the plan and billed to their student account in September and January.
If the student has another plan of comparable coverage, they must present a copy of the policy that proves their coverage within the first two weeks of classes each semester (September and January).

The student must hold a non-immigrant visa and be physically and actively attending classes on campus in order to be enrolled in this plan.
International faculty and visiting international high school students are also eligible for this policy.
