Background Check Forms
- Criminal Background Check Form (Required for Admission to Take TEDU 210 or 230, Admission to the Teacher Education Program or Admission to Student Teaching)
- Disclosure Questionnaire (Required of all students who do not need to complete a Criminal Background Check during the semester)
