Need data? Use the new data request form

The Office of Institutional Effectiveness (OIE), in collaboration with the Data Team for the University of Wisconsin-Parkside, has created a form for all data requests across the university. The Data Team, which includes the Registrar, OIE, Human Resources, Admissions, and Campus Technology Services (CTS) among others, decided that to manage data requests more efficiently and effectively and to ensure requests go to the appropriate office for fulfillment, a data request form would be helpful for the campus.

The data request form is available on the university web site. 

The form was created to:

  • Ensure requests are funneled to the correct person and office
  • Have a record of requests received
  • Obtain the information needed to understand the request, and
  • Ensure as timely a response as possible 

The new form is the place to begin when you have a request that needs student, faculty, or staff-related data. Once the request is received, the staff of the appropriate office will follow up with you to make sure your needs are well understood, and all the necessary information needed to fulfill the request has been identified.
