Parental Consent Form

The university requires that a parental consent form be filled out and submitted when your son arrives at the university on May 28, 2008. Please print out this consent form, sign it and send it along with your son to camp. Of course we look after all finalist with care and have only once left one at an ice cream parlor (for a short time.)

The following form is to be filled out by a parent on guardian of the USACO finalist.

  1. By filling out and signing this form, you are giving your consent in advance for medical treatment at an appropriate medical facility in case of illness or injury.

  2. By filling out and signing this form, you are aware of and accept any risk inherent in the program activity. For example, we play Disc Golf, Ultimate Frisbee, and attend Great America Amusement and Water Park-- besides do programming.

  3. By filling out and sending this form, you agree to hold harmless and indemnify the Board of Regents of the University of Wisconsin System, and the University of Wisconsin - Parkside, their officers, employee and agents, from any and all liability, loss, damages, or expenses which are sustained, or required arising out of the actions of your dependent in the course of the USACO camp.

Finally,

We are looking forward to having your son attend the USACO training camp at the University of Wisconsin-Parkside, May 28 - June 5, 2008.  Our staff will take all precautions to make sure his camp experience is a safe and enjoyable one.

While at the IOI 2008, all expenses are paid for by the host country Egypt. All cost associated with the USACO camp and travel to and from the IOI in Egypt is paid for by the sponsors of the USACO -( USENIX, ITA, and IBM).

Before we invest our time and money in bringing your son to camp, we need to know the following:

"If my son makes the USA team of the top four students, he will be able to travel with the US team to Cairo, Egypt to participate in the International Olympiad in Informatics,August 16-23, 2008."
