To be exempt from the surveillance COVID-19 testing, Parkside students, faculty, and staff must add their Wisconsin Immunization Record or photo of their Vaccine Record Card to their profile in MyChart.

STEP 1: Get Your Record Ready

Grab your original COVID-19 vaccine record card and a smart phone


STEP 2: Log into MyChart

Use your Parkside login credentials, birth date, and DUO (two-step authentication) to log into MyChart. Follow any prompts.

STEP 3: Add Immunization Record

Expand the menu (three white lines) in the upper left corner and select “Immunizations.”

Then tap/click the button “Add Immunization Record...”

Use your phone’s camera to take a picture of your vaccine record card. Place your record card against a solid surface and make sure your card is fully visible.

Tap/click the “Save Records” button

**NOTE: If you had Pfizer or Moderna BOTH doses must be accounted for onour immunications record.**

STEP 1: Get Your Record Ready

Start by visiting the Wisconsin Immunization Registry

NOTE: Illinois residents can visit the Illinois Resident Immunization Portal and follow instructions on the portal site. Once you obtain a PDF, you may skip to step 2.
 

Enter your first name, last name, birth date, and social security number. Please note that this is your legal name.

Select Search

If your vaccination shows up, download (print to PDF) a copy of your immunization record. Save it in a location you can easily find it, like your desktop.


STEP 2: Log into MyChart

Use your Parkside login credentials, birth date, and DUO (two-step authentication) to log into MyChart. Follow any prompts.

STEP 3: Add Immunization Record

Expand the menu (three white lines) in the upper left corner and select “Immunizations.”

Then tap/click the button “Add Immunization Record...”

Upload the PDF of your immunization record from your saved files 

Tap/click the “Save Records” button

NOTE: If you had Pfizer or Moderna, BOTH doses must be accounted for on your immunization record.

Take a look at some of these common challenges and tips to solve them.

Some students, especially those with iPhones, have noted that the picture they take is too large to upload to MyChart. We are working on increasing the file size maximum, but in the meantime, you can temporarily change your phone's camera setting to a lower quality resolution.

You can do this in Settings > Camera > Formats. Selecting "Most Compatible" should solve the problem.

Some students have received error messages indicating that they could not be located in the Wisconsin Vaccination Registry. If this is the case, we recommend taking a photo of your original Vacine Record Card and uploading directly to MyChart. 

If you do not have your vaccine record card, you may be able to receive assistance from the Wisconsin Vaccination Registry by using the "Help" button on their site. 

If you are not sure if you have already uploaded your vaccination record, simply log into MyChart and select "Immunizations" from the left-side menu. (Click the three white lines in the corner to expand the menu.) If you see thumbnail graphics representing files, such as a PDF or a photo, you have already uploaded your record. 

If no files appear, we recommend that you follow the steps outlined for uploading either a photo or a PDF copy of your record to MyChart.

Instead of using the Wisconsin Vaccination Registry, Illinois residents should use the Illinois Resident Immunization Portal to retrieve a PDF copy of their record.

Once you have a PDF copy, the instructions for uploading to MyChart are the same as Wisconsin residents.

NEXT STEPS

The document is verified by Student Health Clinic personnel. Once verified, students, faculty, and staff will receive an email from the clinic notifying them that they are exempt from COVID-19 testing as long as they are asymptomatic.

SHCC Staff will review, verify, and acknowledge the immunization record. After that process, which may take 2-3 business days, you will receive a secure message in MyChart from the clinic staff confirming that you are no longer required to participate in surveillance testing.

Please keep in mind that you may still contract COVID after vaccination. If you are symptomatic, you should seek medical attention. Students can call the Student Health & Counseling Center at 262-595-2366 to make an appointment; faculty and staff should consult their primary care providers.
