Report of Outside Activities, Faculty and Academic Staff Notice to Avoid Conflict of Interest, Classified Staff
This memo will serve as the annual reminder of (1) our UW System policies and obligations for unclassified staff to report on outside activities, (2) our obligation to be familiar with our policy on institutional and employee relationships with educational loan lenders, and (3) the need for classified staff to be familiar with the policy to avoid conflict of interest.
Outside activities reporting is separate from, and in addition to, the annual economic interests reporting requirement applicable to those unclassified staff who are authorized to sign contracts; economic interests reporting is administered by the Board of Regents Office.
The UW System Guidelines for Reporting Outside Activities, the 2012 UWS Report Form and Chapter UWS 8 are attached for your reference. Also attached is Regent Policy Document (RPD) 13-4, University of Wisconsin System Policy on Institutional and Employee Relationships with Educational Loan Lenders and a copy of the UW System Ethics Brochure.

- Return form to your Department Chair/or Director for signature
- Chair/or Director sign and forward to the Dean
- Dean signs and forwards to Human Resources by Monday, April 30th
Classified staff: no additional action needed (you are not required to complete the outside activities report), but ensure you've read the memo and ER-MRS 24.
Unclassified staff: review and fill out outside action report form; print out and submit the completed report to your Department Chair or Director.
